Mebendazole is an anthelmintic medicine used for susceptible intestinal worm infections. These include threadworm (pinworm), roundworm, whipworm and hookworm infections, including certain mixed infections.

These tablets are taken by mouth and may be chewed or swallowed whole, with or without food. For threadworm, the usual regimen for adults and children aged over 2 years is one 100 mg dose; a further dose may be advised after 2 weeks because the medicine does not kill worm eggs. Other worm infections can require a different regimen, so follow the product directions or a clinician's instructions.

Mebendazole should generally be avoided during pregnancy, particularly early pregnancy, unless a clinician decides that treatment is needed. During breastfeeding, small amounts may pass into milk and the individual benefits and risks should be discussed with a pharmacist, GP or other prescriber.

It binds to tubulin inside the worm's cells and disrupts the formation of microtubules needed for glucose uptake. Without an adequate energy supply, the worms die over the following days and are cleared from the bowel.

How Vermox is used

For threadworm, the standard approach is a single 100 mg dose. A pharmacist or doctor may advise repeating the dose after two weeks because mebendazole kills the worms but not their eggs. For roundworm, whipworm, hookworm, and mixed infections, the general regimen is 100 mg twice daily for three consecutive days. Follow the dosing schedule in the patient information leaflet or the instructions given by a pharmacist or GP, as the right course depends on the worm involved, age, previous treatment, and other individual circumstances.

The tablet can be chewed or swallowed whole and may be taken with or without food. For a young child who cannot chew or swallow it safely, the tablet may need to be crushed; ask a pharmacist or doctor for guidance and supervise administration. Laxatives and changes to diet are not normally needed. Hygiene measures such as washing hands, keeping nails short, cleaning bathroom surfaces, and laundering nightwear and bedding help reduce reinfection. When one person has threadworm, treatment may be advised for all eligible household members at the same time.

Onset and what to expect

Mebendazole starts acting after it is taken, but it can take several days to kill the worms. You may not see worms in the stools after treatment, and threadworm eggs are microscopic. Symptoms such as itching around the anus should improve as the infection clears, although persistent or returning symptoms require advice from a pharmacist or doctor.

When the type of worm matters

Not every digestive symptom is caused by threadworm, and mebendazole is not the preferred treatment for every parasite. Seek medical advice if the type of infection is uncertain, symptoms are severe, there is unexplained weight loss or blood in the stools, treatment has not worked, or the infection may have been acquired during travel. A clinician may recommend testing or a different anthelmintic when another parasite is suspected.

Safety, interactions, and side effects

Vermox is generally well tolerated at standard doses. Abdominal pain is among the more commonly reported effects, while abdominal discomfort, diarrhoea, and flatulence can also occur. Hypersensitivity reactions are uncommon but possible, and anyone with known hypersensitivity to mebendazole or another ingredient in the tablet should not take it. Stop taking the medicine and seek urgent help for facial swelling, difficulty breathing, or a severe blistering or peeling rash.

Who should take particular care

Mebendazole is not recommended during pregnancy. Animal studies showed embryotoxic and teratogenic effects at high doses, so treatment for threadworm during pregnancy is usually deferred in favour of hygiene measures or discussed individually with a doctor or pharmacist. Mebendazole is poorly absorbed from the gut and only very small amounts appear in breast milk, but it should still be used with care while breastfeeding and only when clearly needed. Mebendazole 100 mg is established for children aged 2 years and over; use below this age requires particular benefit-risk consideration because convulsions have been reported in very young children.

Interactions

Mebendazole has few clinically significant interactions during the short courses used for common intestinal worm infections because little of the medicine is absorbed into the bloodstream. Concurrent use with metronidazole is generally avoided, as this combination has been linked to rare but serious skin reactions. Cimetidine can reduce the breakdown of mebendazole and increase its concentration in the blood, particularly during prolonged or high-dose treatment. Tell a pharmacist or doctor about all regular medicines before starting treatment.

Monitoring and when to seek advice

Routine blood tests are not normally required for a standard short course. Prolonged or high-dose treatment is different and may require medical supervision, liver-function tests, and blood counts because rare liver inflammation and blood-cell disorders have been reported. Contact a doctor promptly if treatment is followed by yellowing of the skin or eyes, dark urine, unusual tiredness, unexplained bruising, fever, or a persistent sore throat. Suspected side effects can also be reported through the MHRA Yellow Card Scheme.

Storage

Vermox tablets should be stored at 20°C to 25°C, with occasional excursions between 15°C and 30°C considered acceptable. Keep the tablets in their original packaging, protected from direct light and moisture, and out of the sight and reach of children.
