How Mebendazole Works and How to Take It

Mebendazole interferes with the parasite's ability to build microtubules, the internal structural fibers that worms depend on for movement and for absorbing glucose. Without functioning microtubules, the worm can no longer take in the sugars it needs to survive and is eventually cleared from the intestine in the stool over the following days.

General Regimen

For pinworm, a single 100 mg dose is commonly used, with a repeat dose after two weeks often recommended because pinworm eggs can survive on surfaces and reinfect the same person. For roundworm, whipworm, hookworm, and mixed infections, a 100 mg dose taken twice daily, morning and evening, for three consecutive days is a typical general regimen. These are established treatment patterns rather than a specific dose we are assigning to any individual reader; an appropriate regimen always depends on the infection identified and a clinician's assessment.

The tablet can be chewed, swallowed whole, or crushed and mixed with food, and it may be taken with or without food. Taking it with a fatty meal increases how much mebendazole the body absorbs, which is not usually a concern for a short intestinal-worm course but is worth knowing if a longer course is prescribed for a different indication.

The number of tablets needed for a treatment course can be much smaller than these package quantities and depends on the diagnosed infection, whether repeat dosing is advised, and how many people a clinician recommends treating. Do not treat household members or repeat a course solely because tablets remain in the package.

Safety, Interactions, and Who Should Be Cautious

Mebendazole is contraindicated for someone with a known hypersensitivity to mebendazole or to another ingredient in the tablet; an unrelated or unspecified allergy history is not on its own a reason to avoid it. It has no recognized restriction related to diabetes and is not expected to cause drowsiness or affect the ability to drive or operate machinery.

Pregnancy, Breastfeeding, and Children

Because animal studies have shown embryo-fetal effects even though available human pregnancy data have not linked mebendazole to major birth defects or miscarriage, use during pregnancy calls for an individualized decision that weighs the infection against these findings, rather than an automatic yes or no. Small amounts of mebendazole pass into breast milk, and because effects on a nursing infant are not fully known, breastfeeding parents and their clinicians should weigh the treatment need against this caution. Pediatric use is established for children age 2 and older. Safety and effectiveness have not been established below age 2, and seizures have been reported in infants, including those younger than 1 year.

Drug Interactions and Monitoring

Carbamazepine and phenytoin can lower mebendazole levels in the blood by speeding up its breakdown, which may reduce its effectiveness against certain infections, while cimetidine can raise mebendazole levels by slowing that breakdown. Combining mebendazole with metronidazole has been associated with rare serious skin reactions, so this pairing is generally avoided. For short courses, routine laboratory monitoring is usually unnecessary, but a clinician may check liver function and blood cell counts when mebendazole is used at higher doses or for longer than a standard intestinal-worm treatment course.

Common and Serious Side Effects

At the doses used for intestinal worm infections, mebendazole is generally well tolerated. Commonly reported effects include abdominal pain, diarrhea, gas, nausea, vomiting, and headache. Rare but more serious effects that warrant prompt medical attention include unusual bruising or bleeding, signs of low blood cell counts such as persistent fever or infection, yellowing of the skin or eyes, and any severe skin or allergic reaction.
